Peroxide does not just sit on the surface. Over-the-counter products cover strips, paint-on gels, prefilled generic trays, rinses, and gums. Peroxide levels sit below what a dentist dispenses; they are built for unsupervised use, and quality varies. Sensitivity, and What Can Be Done About It
Temporary tooth sensitivity is the most common side effect of every form of bleaching. It shows up as short zings with cold air or cold drinks, and the literature describes it as mild to moderate and transient, affecting up to two-thirds of users early in a course. With strips and trays, it often starts two to three days in and settles within a few days of finishing. The leading explanation is temporary inflammation in the pulp as peroxide passes through.
Gum irritation is the other common complaint, and it usually traces to delivery rather than the gel itself: a poorly fitting tray, a strip pushed too far up, or a barrier that was not sealed properly. It also tends to resolve shortly after treatment ends.
Supervision is where the paths separate most sharply. In a dental office, the tissue is monitored throughout, and dentists are advised not to numb patients for whitening, so any burning sensation can be reported and the barrier rechecked. Approaches to blunting sensitivity have been studied, including gels containing potassium nitrate and fluoride, though the evidence supporting them is not settled. At home with a drugstore kit, the interpretation of that discomfort falls to you.
What Whitening Cannot Change
Only natural tooth structure responds to peroxide. Crowns, veneers, bonded fillings, and other tooth-colored restorations retain their original shade, so lightening the surrounding teeth can create a visible mismatch. Front teeth restored years ago are where that mismatch tends to show up.
Certain intrinsic stains respond slowly or unevenly. Brown discoloration from fluorosis or tetracycline exposure can lighten, though tetracycline staining has been reported to need three to four months of nightly treatment. White spots from fluorosis or a past orthodontic appliance do not bleach away; they often become less noticeable as the surrounding tooth lightens, though sometimes more obvious at first.
